TRANSCRIPT REQUEST FORM
This form is to be submitted ONLY by students CURRENTLY ATTENDING Marquette High School.
***ATTENTION: YOU MUST HAVE THE FERPA SECTION COMPLETED IN SCOIR BEFORE WE CAN PROCESS YOUR REQUEST(S).***
This form, which must be completed from a student’s Rockwood Google account, is used to request that your “official” transcript be sent to ONE college, university, athletic eligibility center, coach, scholarship, etc. If you need your transcript sent to 3 places then you will complete this form 3 separate times. We require a minimum of 10 school days for requests to be processed and keep in mind that after we send your transcript, it can still take another 2-3 weeks for the recipient to process it on their end. Note: Students & Parents have access to the “unofficial” transcript in Infinite Campus.
IMPORTANT for students using CommonApp to apply: You must add your College and Career Counselor as your "counselor recommender" in the FERPA/Recommenders section.
MID-YEAR TRANSCRIPT REQUESTS
Some colleges and universities require a mid-year (7th semester) transcript as part of the application process. If a school you have applied to falls into this category, and you have already requested your original transcript, then please use the Mid-Year Transcript Request Form to ask us to send the updated transcript after the end of the first semester.
Former students should contact the following to request transcripts:
Students who have moved or graduated from Rockwood School District within the last year should request school records/transcripts from their last school of attendance.
We must have a FERPA Release on file to send transcripts on behalf of former students.

Class of 2021 and before (those who have graduated/dropped) may request transcripts by contacting the Rockwood Permanent Records Office at the following address:

Requests are processed Monday through Friday between 7:30 a.m. and 4:00 p.m.
You may send a written request to the address above with the following information:
- Name as it appears on school records
- Date of birth
- School last attended or school of graduation
- Year of graduation or year last attended
- Address where the transcript/record should be mailed
- Phone number where you may be contacted
If you are making a request for either student records or transcripts for someone other than yourself, please include a signed letter of consent for release in order to obtain the records.
