The process of enrolling a student in the Wellness Center will take the following form:

• Referral: A form to be completed by the student, school or a family member
• Family Meeting: A personalized conversation with the staff at the Wellness Center to understand the family's needs and how the center can best meet those needs
• Wellness Center Review Team: The referral application will be reviewed by the director, clinical specialist, nurse and counselor to determine if the center is the right fit for the student
• Enrollment: The center will formally notify the family of enrollment and work with school and family on transitioning to the center
