The Visiting Nurses Association will be providing state-required vaccines for our students.  

These immunizations, which are required by Aug. 22, will be billed to your health insurance or provided free of charge if your child is not currently covered by insurance. Please complete this consent form (en Español) and bring your insurance information to the vaccine clinic.

Clinic Options

• Eureka High: Tuesday, Aug. 9, 1-3p.m. in the atrium (main entry to the building)
• Marquette High: Tuesday, August 16, 5-7 p.m. in the theater lobby (main entrance, to the left)

Students under age 18 cannot be vaccinated without signed parental consent. Visit the Missouri Department of Health and Senior Services site for more information on required immunizations for the 2022-2023 school year.
